Radiology reporting standard

TI-RADS

ACR TI-RADS is a point-based system for thyroid nodules on ultrasound: points for composition, echogenicity, shape, margin and echogenic foci add up to a level from TR1 to TR5, and the level plus the nodule's size decides whether fine-needle aspiration or follow-up is recommended.

How RadVector applies it

TI-RADS, applied the same way every time.

The same deterministic engine grades TI-RADS inside RadVector Reporting and through RadVector API, and a test keeps the two answers identical.

  • Adds the points for the features the report documents and, when a feature is not documented, states the range the nodule could reach (for example TR4 at minimum, up to TR5) instead of guessing.
  • Grades numbered nodules one at a time, sizes by the largest axis, and applies FNA and follow-up size thresholds from the pack.
  • Treats a spongiform or purely cystic nodule described beside a suspicious feature as a contradiction to resolve, not a score.
  • Checks the finished report: a nodule at its FNA threshold with no FNA recommended, or FNA recommended below threshold, is flagged for review.

Radiologists and imaging centers

RadVector Reporting supplies the TI-RADS category and recommendation to the report draft from the radiologist's own findings, cites the guidance applied, and keeps a per-report governance record. The radiologist signs.

TI-RADS questions

What does RadVector do when a TI-RADS feature is missing from the report?

It does not assume one. RadVector adds the points for what is documented and reports the range of TI-RADS levels the nodule could still reach, naming the undocumented feature, so the radiologist can complete the description. Only features reported by exception, such as echogenic foci when none are mentioned, take a stated pack default, and the answer says so.

Is there a TI-RADS calculator API for software?

Yes. RadVector API returns the TI-RADS level, the recommendation and the provenance for each nodule, and accepts TI-RADS features as structured fields or free text. The TI-RADS pack is a draft under validation, so answers are advisory and marked not usable for output until validation is complete.

Other reporting standards

RadVector is clinical decision support for qualified professionals, not a diagnostic device; the radiologist makes the final determination.
