Radiology reporting standard

Lung-RADS

Lung-RADS is the American College of Radiology system for reporting and managing findings on low-dose CT lung cancer screening, assigning each screening exam a category from 0 to 4X that sets the next step in screening.

Applies to
Low-dose CT performed for lung cancer screening. Incidental nodules found on other chest CT are managed with Fleischner, not Lung-RADS.
Modality
Low-dose chest CT (screening)

Lung-RADS categories

0
Incomplete: prior studies or part of the lungs needed, or findings suggesting infection or inflammation.
1
Negative: no nodules, or nodules with clearly benign features.
2
Benign appearance or behavior: continue annual screening.
3
Probably benign: short-interval follow-up CT.
4A / 4B
Suspicious and very suspicious: closer follow-up or further workup such as PET/CT or tissue sampling.
4X
Category 3 or 4 nodule with additional features that raise suspicion.
S
Modifier for clinically significant findings other than lung cancer.

How RadVector applies it

Lung-RADS, applied the same way every time.

The same deterministic engine grades Lung-RADS inside RadVector Reporting and through RadVector API, and a test keeps the two answers identical.

  • Applies only to an exam the report identifies as lung cancer screening; an incidental nodule on a diagnostic chest CT is routed to Fleischner instead.
  • Sizes each nodule as the mean diameter to 0.1 mm, keeps a part-solid nodule's solid component separate, and reads growth against comparable prior CT only.
  • Recognizes category 0 situations (incomplete or infectious) and a lung mass, and never treats a version number or template legend as the stated category.
  • Every answer names the pack, edition and rule version; a deferral names the missing feature.

Radiologists and imaging centers

RadVector Reporting supplies the Lung-RADS category and recommendation to the report draft from the radiologist's own findings, cites the guidance applied, and keeps a per-report governance record. The radiologist signs.

Lung-RADS questions

What is the difference between Lung-RADS and Fleischner?

Lung-RADS governs low-dose CT performed for lung cancer screening. The Fleischner Society 2017 guidelines govern pulmonary nodules found incidentally on CT done for another reason, in adults 35 and older without known cancer or immunosuppression. RadVector chooses between them from the exam the report names, so a screening nodule is not given a Fleischner interval and an incidental nodule is not given a Lung-RADS category.

Is there a Lung-RADS calculator API?

Yes. RadVector API returns the Lung-RADS category for structured or free-text screening findings, with the criteria pack and version that produced it. Unlike a one-off web calculator, every answer is versioned and carries a trace id. The Lung-RADS pack is a draft under validation, so today the API answers as advisory and marks the result not usable for output.

Does RadVector Reporting assign Lung-RADS in the report?

RadVector Reporting reads the radiologist's findings on a screening CT and supplies the Lung-RADS category and recommendation as decision support to the report draft. The radiologist reviews, edits and signs every report.

Other reporting standards

RadVector is clinical decision support for qualified professionals, not a diagnostic device; the radiologist makes the final determination.
