Radiology reporting standard

Bosniak

The Bosniak classification, updated in version 2019, sorts cystic renal masses on CT and MRI into classes I, II, IIF, III and IV by wall, septa, enhancement, mural nodules and attenuation, from simple benign cysts to masses likely to be malignant.

Bosniak categories

I
Simple cyst: benign.

Benign cyst with minimal features, or a homogeneous lesion in defined attenuation ranges.
IIF
Probably benign: imaging follow-up.
III
Indeterminate: thick or irregular enhancing wall or septa.
IV
Enhancing nodule: likely malignant.

How RadVector applies it

Bosniak, applied the same way every time.

The same deterministic engine grades Bosniak inside RadVector Reporting and through RadVector API, and a test keeps the two answers identical.

  • Grades any described renal cyst on any exam, including a cyst seen in the upper abdomen on a chest CT.
  • Reads Hounsfield units by phase: simple fluid, the class II attenuation bands, and a pre and post contrast pair that measures enhancement itself.
  • When an attenuation is not encoded by the classification, names enhancement and a renal mass protocol CT or MRI as what would decide it.

Radiologists and imaging centers

RadVector Reporting supplies the Bosniak category and recommendation to the report draft from the radiologist's own findings, cites the guidance applied, and keeps a per-report governance record. The radiologist signs.

Bosniak questions

Can RadVector assign a Bosniak class from a report sentence?

Yes, as decision support. RadVector reads the cyst's wall, septa, enhancement, nodules and Hounsfield units from the radiologist's description and returns the Bosniak 2019 class, or names the feature it needs. The Bosniak pack is a draft under validation, so the result is advisory.

What does RadVector do with a low-density renal mass of 30 to 70 HU?

Version 2019 does not assign a class to every attenuation. When a value falls outside the ranges the classification encodes, RadVector defers and names enhancement measurement and a renal mass protocol CT or MRI as the step that would decide it.

Other reporting standards

RadVector is clinical decision support for qualified professionals, not a diagnostic device; the radiologist makes the final determination.
